How do I add a counter to my form?

I want to add a submission counter that puts a sequential number in a hidden field...starting with number 1 (one) then, 2,3,4,5 etc...  So the first form submission would have the number 1 in the hidden field.  The second submission would have the number 2 in the field.  How do I accomplish this?
