When rules enabled, cannot see form details

Hello,

When setting rules on my form, the fields disappear and the form comes up blank. If I remove the two rules that deal with the Transport/ first Responder rule, the form data can be seen again.

I checked the related form and I think you have figured it out how to overcome from that situation; but if you are still thinking that why most of my fields are staying hidden when there is no selection on "Transport or First Responder Call" field, you need to modify or delete the third condition from the image below:
