Jotform has been disabled once published

    Hi all,
     
    I have just created a quick form in regards to Gfinity Online in order to receive applications for FIFA 16 as an admin. Unfortunately once the form was published it was disabled and isn't viewable to other people. Can this be resolved please? The form I created is located below.

    Your form was disabled because our anti-Phishing system detected one of your fields is asking for a username.

    Please note that we do not allow forms asking for sensitive or critical information like log in credentials, credit card details, bank information and the likes. This is a violation of our terms of use which will only result to your account being suspended. Please check our terms page specifically the "Phishing" section.

    For now, I have deleted that field in your form and re-activated your account. Please do avoid creating similar forms.
