    Your form was disabled because our anti-Phishing system detected one of your fields is asking for a username.

    Please note that we do not allow forms asking for sensitive or critical information like log in credentials, credit card details, bank information and the likes. This is a violation of our terms of use which will only result to your account being suspended. Please check our terms page specifically the "Phishing" section.

    For now, I have deleted that field in your form and re-activated your account. Please do avoid creating similar forms.
