We are using the ADD OPTIONS widget on our form but it has a glitch when the enter button is pushed. I add a name and then hit the enter button and the entire text box disappears. Please let us know how this issue can be fixed.

I just tested your form, and I did not find any issues:
Have you embedded your form in a webpage? If that is the case, please provide the URL of the page where your form is embedded, so we can test there.
The issue is not that I can't type names. The issue is that when the first name is typed and the person then presses enter on their keyboard rather then the add button the firm is then messed up. Thank you for looking into this!

I did a test on a your form and on another one I created and was able to recreate the issue.
This is what happens, when you enter a value on the first option field and press the enter button the form is reloaded with the field removed and when add is clicked no field is added back.
I will forward this to our development team for investigation into the cause, meanwhile you can add a footnote instructing the users not to press the enter key when adding members.
Thank you for your patience - the widget is now fixed - the values will not be cleared if you hit the enter button.
