I have the same problem as well and all i'm using for my forms is for clients websites that i'm creating

Upon checking your account I found that it's suspended by our anti-phishing system. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities.

Upon taking a closer look at your form, I can confirm that it's not being used for any phishing activities. I have activated your account now. Do try to sign in to your account or test your forms and get back to us if the issue persists.
