Account suspended?

    my jotform user name is talktodan, i  have been suspended and i do not know why i am using jotform to create a form and i've only set up a form yet. It would help me greatly if you could quickly allow me access to my account. please and thank you in advance.

    I checked your account and form, your account is suspended because the form titled "Beneficiary". Your form is asking for sensitive and critical information like bank details, passwords, usernames and the likes. Please do take note that this is not allowed in our forms and is a violation of our terms of use. Please check our terms, especially the "Phishing" section.

    We can re-activate your account but we will need to delete or edit the said form, after that, you will need to agree that you will not create similar forms in the future as this will only result to your account being suspended.

    We'll wait for your response.
