How can I create a multipage form?

Also, how can I style the form like that sample? I like the page bar at the top

I've checked the form you provided and I can see it uses images to show the progress bar at the top, refer to this guide to add images to your forms :

You can choose the way you want to upload the image to your form : 

Do note, in the form you share us, are 4 pages, then there are 4 different images showing the progress, now if you would like to add more than 4 pages, you will have to customize the image.

Now, to do Multi Pages Forms, do it easily following this guide :
