I setup the Folder in Google Drive, but how do I add it to the end of the form?
After they fill out the form I want them to have the option of attaching a file.

You are currently using a Google Form in the given website.
I would suggest creating the form in Jotform then you can add the file upload field to allow someone to upload a file in your form. Here are our guides:
If you want to integrate your form to Google Drive, that is also possible. Simply check this guide: http://www.jotform.com/help/192-How-to-Integrate-a-Form-with-Google-Drive.

I presume that you are asking if the jotform you create now will be linked to the spreadsheet that you had with your Google Form.
If that is the case, then the answer would be no.
Each time you create an integration from your jotform to Google Spreadsheet, a new spreadsheet is created for that form.
