I'm getting an email from a different form

I sent a customer an HD Order Form with this link: https://form.jotform.co/53309027917861.  However what I received back from the customer was an FD Order Form which is this link: https://form.jotform.co/53202972278862.

I thought I had made a mistake but when I checked the URL in the email I sent to the customer, I see that I had sent the correct order form which is the URL that ends with 861.  So why did the FD Order go to the customer?

Also, how do I change what reads at the top of the window and tab (which reads: HOMEplate Online Order Form)?  See the screenshot to see what I mean.

You just need to update the email notifier/autoresponder to include the correct fields. I would suggest doing it this way:

    1. Delete the current notifier/autoresponder: http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder.

    2. Re-create it: http://www.jotform.com/help/25-Setting-Up-Email-Notifications and http://www.jotform.com/help/26-Creating-a-Form-Autoresponder.

Your last question about the form title has been moved here: http://www.jotform.com/answers/713519.

    Thanks

    Hi Welvin,

I did exactly as you said to do but that did not fix the problem.  Notice that when clicking on Notification, the list of Form Fields on the right are numbered but the ones in the larger box are not.  What's in that larger box shows the list of form fields I had created in my very first form, which was the FD form and I did not number them.  How do I get the Form Fields showing in the right column to match up with those in the larger box?

I'm sure it's something simple, but I just don't see what I'm doing wrong at present.  I just sent out an EZ Order Form, and that, too, came back as the FD Order Form.  I must be missing a simple step but I don't know what that is.

You can fix it by re-creating the whole notifier. I have already given you the guides. Would you mind if I'll do it for you? Let us know.
