The form sub-headings don't transpose leaving gaps in the fields

On another note I'm experiencing a number of other problems with the fillable form. The form sub-headings don't transpose leaving 'gaps'.

I think you mean the following, for example:

There is a text block between the dropdowns. You can see a gap, but looking at the form, the spacing looks the same. You can try changing the spacing in the form and see if that fixes it.
