Why cant users complete my form?
i read some more on other quesitions and it says you cannot ask for passwords but my form contains stuff me and my team created, the beta password is something we use to see if they are in fact beta testers for my teams software production, if that is the reason my form cannot work.
All the things my form ask for is not information form other people just my team and website.

Sorry about that. I have now unsuspended and white listed your account, so you can now login normally.
