submit button does not work on pdf form

I am testing the form and created a pdf form of a questionnaire but after filling out the form in PREVIEW (Mac), I pressed SUBMIT and nothing happens.

I should advice you that if you are trying to submit the form using a Web Browser, it's not possible, you have to do it using a PDF reader like Adobe Reader or Foxit Reader, I cloned your form and I was able to submit it.

I'm using Adobe Reader,please try submitting the form in some of the PDF readers metioned above and let us know if it works.
