I'm not able to create the pdf from the ipad app to make this work offline.. Help?

You will need to log in to your www.jotform.com, then click "Create PDF form". If you would like to create the fillable PDF form in the JotForm iPad/iPhone App, unfortunately, it does not have that feature. The JotForm iOS app is usually for viewing your submissions and getting notifications.
