Based on the therapist name on my form I want to populate the email field with their email address and upon submit have an auto response send them an email with the form information for their records.
Cannot seem to get the auto responder to send an email.
Right now the only email I have populating is for therapist claussen. I am having to hard code email based on response. Is there a better way?

If I can understand, you are trying to send the Autoresponder to the Therapist email selected, now I cloned your form and I can see your condition is working fine, I was able to receive the email notification when submitted the form, I would suggest you to follow this guide in order to send the email to the correct recipient based on the selection :
Instead of use notifications you will use the Autoresponder, now I also would suggest you to ask the email on the form , since I see there is not any email field, then re-configure the Autoresponder and select as recipient email the field :
Doing it by this way , you will prevent to use multiple conditions.
