I run a residential and commercial cleaning business.
I use a backend software called getjobber that's great for my backend but doesn't do much for the web user experience.
I want my form to pop and be a lot less boring while staying user friendly.
I would love to use icons and images instead of text for my add ons.I want it to look more like this:

I wonder if anyone on this forum can do this?

We can guide you in the process of designing your form, but if what you want is a ready product it is better to use a freelancer platform like Upwork, or Guru (to name a few) to hire a designer.
