Why I can't visit the page directly?

Dear sir,

I embeded a form at http://www.2blife.com/form/sgig_xmas2015.html .

When I visit the page it show as below picture

Then I click the link,

It will show the form as below picture (it is a okay picture),

I want to ask how can I visit the page directly under http://www.2blife.com/form/sgig_xmas2015.html?
