We received our first submission on our form and one of the notifications receivers did not receive his email. I have since sent two test submissions and no one received an email. When I test the emails, they work.

The users who have submitted your form didn't receive any email because you have only set a "Notification" to your form and this only sends a email submission to the owner of the form.
To send a confirmation email (Autoresponder) to your users, you need to set a "Autoresponder" to your form. Below, you will find a instruction about how to do it:
