How to Create a Secure Form to Collect Medical Data?

    Hi -

     

    I am working with a dentist who is wanting me to develop a medical history form form that new dental clients can type in their medical history and have the form supmit for the Dentist to review. 

     

    I also wish to offer a way for clients to make payments online.  

     

    Could you call me and go over the details of how I can make this work for my client?

    Recently, JotForm completely moved to secure SSL, so you don't need to worry about the secure connection anymore. You can find the detailed post about this feature from the link below:
