How can I create an appointment form?

I used the "Checklist" widget to create an Appointment form that has all of my available time slots on display for my clients to "Check" the time slot they would like. How do I create an appointment list that has the capabilities of scratching off that appointment time slot when booked? I have tried several different style widgets for lists and I think I am just over looking the one I need! Any help would be greatly appreciated :)

Kindly take a look at the multiselect grid this allows you to specify the time slots in a grid like layout and when one is selected it will be flagged and the next user wont be able to select that slot again.
