The account username of the form is: theflyerkid, and the email address used to register the account is: mo@theflyerkid.com

If you have access to that email, simply click on reset password, and the instruction will be sent there. If you don't have access to it, the owner of the form is the only allowed to give access to his forms.

    Hi,

We can move the form you need from one account to another. We can move the form from account theflyerkid to your account iydsouthernregion. However, you will need to ask permission to the owner of the form for security purposes. The owner of the form must be the one to request the move of the form to your account. Please do contact the owner of the account theflyerkid to give you access to the form.

Do let us know if you need further assistance.
    Regards.

You can try to clone the form http://form.jotform.us/form/30405296579158. That way, you will get a copy of the form to your account.

    Hi,

No, you don't have to pay for just cloning the form. Since you are in Free plan, the following limitations will apply to your account regardless of how many forms (cloned or not) you have.

Yes, you will be able to edit the cloned form. You can consider it yours.

    Hi,

I checked the account setting of theflyerkid  and the Privacy setting for cloning form is enabled.

It looks like the owner of the form does not want other user to clone his/her form. The only way now is to personally contact the owner of the form and ask for the permission to the owner of the account.

    Regards.
