I can't see my header image

Hi: 

I uploaded a header image but I cannot see it in the preview. 

I also tried to upload an existing image, but I still am not able to see the image in the preview. 

Can you help?

I checked your form and I was able to see the header image without issue:

Is it still not showing properly on your end?  If it is not, let us know and we will be happy to have another look.
