I changed the email address for this account (DBHSHLI) from info@dabeka.com
to integratedcare.msm.edu. However, I created the form for this client
and they need modification to the form and the password I have is no longer
working. Can you re-set the Account login email to info@dabeka.comso that
I can reset the password in order to access the form to make changes they
need. Thanks. -

Hi,
Does the user you created the account for no longer have access? They can change the log in information or update the email address quite easily from their account settings:
https://www.jotform.com/myaccount/
The password reset email for that account can also be sent to "integratedcare@msm.edu".
