Form Disabled This form has been disabled

Hello, I recently joined Jotform free account as I have been looking for a way to use forms on my website. Think I managed to set up a few forms OK but I keep getting the form disabled box as above. 

My website forms page is http://thomson-caravans.co.uk/contact.htm This seems to be happening on all 4 forms.

Can you please advise what the problem is, more than likely something I havedone.

Sorry for this inconvenience. I have just verified that your forms did not violate our terms. Everything seems fine and have re-activated your account. You should no longer have any issues viewing your forms.
