I checked your form and while checked the email template I found you were using a different filed before to show the dates, and then you changed to use the widget, in this transition you forgot to change the field on the email template as well, this is the reason why you are not receiving the answers in the widget on your email notifications :
I would suggest you to replace the current field in the email with the new one from the widget, in order to get the updated answer for each submission.
