can i use a W-2 form and make it a fillable form on my website

    See Attached w-2. i would like to be able to put the exact form on my website, set it so that it can be filled out and the input data flow to reports so that i can export it to excel.

    Hi Mike,

     

    You can create a form in JotForm and then embed it to your site. You can achieve the form you want by adding a input fields such as text box to your form. You can check this article for more information: How to Create Your First Web Form. After you have created the form, you can embed it to your website by using the form's embed code. For more information, you can check this article: Which Form Embed Code Should I Use.
