I created a new form and I believe the issue was that I requested people include their old username and password in the form...the form was for people requesting updated username information.
I can remove that line if that is not permitted.

Actually we do not permit the collection of usernames and password using the forms as this is deemed as sensitive information.
You can refer to the terms of service on what type of data you should avoid collecting.
Also you should refrain using trigger words like Login, Password in your forms as this can cause the anti-phishing system to flag your forms as being used for phishing.
Kindly revise your form (Color Disk Login Request) to exclude the prohibited fields and labels so that your account can be re-activated.

Thanks for the quick response and for the information. I have removed the username and password information from the form and have saved it. Please let me know what I need to do to get the account reactivated

I have checked you form and and it appears okay now, I have reactivated your account and your forms should now be normally accessible.
An additional action you can undertake is to rename you form title to exclude the word "Login", this will minimize false flags.
