To add a PDF function to your form, you need to enable the "Print Button".
By using this button, user can get the current state with inputs of your form in PDF format. If you are trying to point different spot, please let us know.

In addition, if you are asking for the "Create a PDF" in "MY FORMS", you can easily access it by clicking "More" after selecting the desired form.
