I want the format to look like this:
Mr Pac Man
123 Awesom pl
Bronx, NY 10459
Re: being awesome
But I only know how to make it look like this:

Unfortunately, I can not understand what are you trying to do, are yo referring to display the full line ?
I see the text is being displayed in a single line, refer to the image below, in yellow the single line, do you want to display the line in different lines as the text in the green box ?

I have my form connected to my google sheets. this info is then transfer to
a letter template. So the address comes out like this:
I cloned your form and integrated with Google Spreadsheet and I can see the data are being sent in separate fields, this is the address field in the form :
And this is how I'm getting the data in the spreadsheet :

yes that works fine. but i want this same thing to happen with the drop
down list. The drop down list has addresses, and i want them to come out
like the way you explain
You can try to add a <br/> tag for the next line in your dropdown option value. Example:
Enhanced Recovery 8014 Bayberry Rd<br>Jacksonville FL 32256-7412 Via Certifiedmail and Fax: (904) 645-3009,<br><br>Derison Puntier<br>733 Mary st 45<br>Bronx NY 10459 Via<br>Certified mail and FAX (917) 979-5781

However, the text displayed in the dropdown is in one line only.
But when you integrate it to google spreadsheet, the data transferred to the spreadsheet will be displayed in multiple lines.
