The question we ask on our rental application about smoking does not show up once someone submits the application on our website.

When someone clicks on the link to complete our rental application form and answers all the questions, the question "Do you or anyone in your party smoke?" Does not show up on the form we receive via email after the applicant hits submit. 

I would recommend you to re-create the email notification, here is how:
