    Hi,

Unfortunately, we do not have this kind of feature in JotForm as we do not recommend our user to add a password field in their form since this is against our terms of use. If you can provide to us the logic why you are adding a password field in your form so we can suggest a workaround?

Do let us know if you need further assistance.
    Regards.

Because I have a website. In this website I have members. I make this form to have their information (name, address, phone number, email, etc...). I have a place where they have to choose a username with a password to log in their profile page in my website.

Unfortunately, we do not allow our user to have a username and password field combination in their form as this is against our terms of use since this kind of information collected might be used for phishing activities. Please do remove the username and password fields in your form to avoid your account to be suspended.

Like my colleague mentioned it is not possible to create forms to collect login credential, like password and username.

Phishing You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

I have checked you account and it is suspended because you have password fields and username fields inside of your forms.

You need to deleted mentioned fields from your forms and only then you account will be activated.

If you are trying to create a sign up or log in form, this is unfortunately not possible with JotForm.  Since the forms cannot query outside sources, the forms would simply collect usernames and passwords, which is against our terms of use.  You can setup a form to collect details such as name, address, email address and other non-sensitive information.

I've deleted those fields and reactivated your account back.

I suggest you to ask username and password with different page which is not related with Jotform. Otherwise, your account will autosespended by our system again.
