select multiple checkboxes

I am not sure if I have understood your question correctly. I did check the webpage URL where you have embedded your form and it does allow multiple selection of checkboxes. Please check the screenshot below:

Yes, it does allow for selecting multiples however, when the form is
submitted, it only shows one selection, not multiple.
