After trying numerous times I am unable to get my form to look how i want it too, I have tried various CSS coding methods and cloned other forms, refreshed my browser and cleared caches. I have tried all suggestions on the forum, but either my exsisting CSS coding is overwriting something, or I am not doing something obvious.
I hope someone can help. I have attached pictures of what I am after below,
There seems to be error in your CSS code. I cloned it and corrected in my end. I also added additional ones to get the layout you want.

You can clone mine and see the CSS code there. Here's a guide on how to clone my form: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL. You can copy the CSS code under the CSS tab in your Form Designer Tool.
I also checked your website and there seems to be a problem on how you have embedded it there, have you tried using our iFrame embed code instead? Here's a guide on how to get the iframe code: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

I noticed there was a huge gap between the "Order Details" and on the product item, so I edited the CSS code again. You can directly see the update on my live clone form.
