On my reservation form, when I receive an email it always has the same person's name showing who from. It did this before and it was never fixed.

You have selected the "Passenger Name" field as your Sender Name so this should be unique for every submission. But I think you are seeing the emails in a thread so I would suggest putting the name field in your subject line. Example:
This way, you will receive each submission as a new thread instead of just a single thread with the same subject line (New Reservation).
