What's the best method for adding multiple fields to a form with a "+" or single click?

Need a referral form for multiple Name and Emails.

Example:

Send us your referrals!

Full name

Email

Add another referral? if clicked then add "Full name 2" and "Email 2"

Add another referral? if clicked then add "Full name 3" and "Email 3"

Etc... 

Submit
