can I import a PDF file to create a form?

I have a PDF Shipping Document that we want to move to an online form. Is there a way to use the existing PDF document as a template to create the online form?

Yes, that's possible. Simply head to this page http://www.jotform.com/adobe-formscentral-alternative/ click on Import Your Forms button and select PDF Only. Then upload your PDF form afterwards. You will then be informed through email that your forms are already available in your account. The process may take a while so please allow some time before the forms will be completed and added in your account.
