Не работает кнопки отправки

    Здравствуйте посмотрите пожалуйста на мой сайт была размещена новая функция Иваном Бабкином сначала не на всех страницах открывалась а сейчас открывается но не отправляется телефон форма перезвоним за 30 секунд очень понравилась но кнопка отправки не работает qualityservise.ru

    I've checked your webpage and able to see one form on the home page. I've tried submitting your JotForm and see that the form was submitted without any issue.
