all of my forms say that they are not found

Please help I logged in to edit my previous form and now I receive an error message "form not found"

Your account is disabled due that you are requesting password in plain text fields on your form "CCG Sign Up" , as shown below:

To collect this type of information in plain text field is considered as a violation to our Terms of Use, as this is only allowed through encryption and our forms do not provide this type of method.

I have removed those guilty fields on the form and reactivate your account.
