I have added a notification email address and I receive the TEST emails, but I a not receiving any emails when the form is completed by someone.

I checked your jotform http://www.jotformeu.com/form/53283704249357 and I found the cause of problem.
There was no email address set on the Recipient E-mail box.
Please add the email address you wanted the form submission email to go to.
Let us know if issue persist.
