How do I change the page title of the form?

I have cloned a form from LeadUS Mentoring Program.  While I have retitled my form, when I send the URL link (as you see below) by email, the top of the screen still reads LeadUS Mentoring Program.  How do I change that?

I understand that you want to change the Page title of the form? You can change the page title from the Form Preferences. Please click on Preferences icon while editing your JotForm to open the Form preferences window. Please see the image below :

After changing the Page title, close the settings and save your JotForm.
