My form is not working

I am trying to send me test emails and it isn't loading a file -

I want to use this form for people to order a product and I will then prepare a paypal invoice to send to them.

I am new user.

Do you mean to say that you are not able to upload any file in your form? I did check your last edited form and did not find any issue with that. I was able to upload file and submit your form successfully. Please check the screenshot below:
