    How Do I Send Date and Checkbox Details to Database?

    Hi, I'm working on a form that i have hosted on our domain and i want to send the submit to mysql db. 

    I'm getting this notice Notice: Undefined index: q14_dateOf14[year]

    same thing for the checkbox and one of the select. 

    I urgently need to get it working. Please help
