    I urgently need to get it working. Please help

     <li class="form-line form-line-column form-col-1" data-type="control_checkbox" id="id_1352">

            <label class="form-label form-label-top" id="label_1352" for="input_1352"><strong> Cereals </strong> </label>

            <div id="cid_1352" class="form-input-wide jf-required">

              <div class="form-single-column">

                  <input type="checkbox" class="form-checkbox" id="input_1352_0" name="q1352_cereals[]" value="Maize" />

                  <label id="label_input_1352_0" for="input_1352_0"> Maize </label>

                </span>

    $dob = $_POST['q14_dateOf14[year]']."-".$_POST['q14_dateOf14[month]']."-".$_POST['q14_dateOf14[day]'];

    $cereals = $_POST['q1352_cereals[]'];

    Hi there,

    There is an easy way to send your data to your database. With "Webhooks" integration, you can easily send Post Data like setting a custom Thank You Page URL.

    Below, you will find a guide about how to do it:

    https://www.jotform.com/help/245-How-to-Setup-Webhook-with-JotForm

    Also, you can get more information about JotForm Webhooks integration:

    If you still want to use advanced method, there is also a guide about it:
