    "Feedback Button" form not fully showing and runs off the page - How to correct?

    Added the HTML script for the orange Feedback Button and the button appears on one of my webpages.  However, when I click it the form doesn't show up in the center of the page (while darkening my page) as yours does, here on Jotform.  Instead, while viewing in Firefox the form runs off the center top of the page and only about the bottom third appears.  While viewing in AOL the form runs off the left top corner and only a portion of the form appears.

    Any ideas on how I can correct this?

    My website is built through Yahoo Pagebuilder.

    Can you post the URL of the page? If you look at the source code of the final page, do you see any changes made on the code by Yahoo Page Builder?

    Since I posted, I've found what was causing the problem and corrected it.

    At the bottom corner of my HTML script box there is a place to check that reads"Include this content in the forms layer."  It wasn't ckecked, so I checked it and everything now works and looks as it should.
