Link is not working. Says form is disable, but it is not.

When linking to this form, an error message is given that it is disabled. However, I have checked multiple times and the form is not disabled.

I have checked your forms on your account and everything seems to be fine. I just noticed that there are forms on the account that are just headers and a submit button. I have also seen that the form you have included on your post has the word "Facebook" in it on the header. I have inspected the form and everything seems good.

I just would like to inform you that we have a phishing detector that scans every form on every user's account. If there are any forms that might be used or suspected of any phishing activity, like stealing login credentials or any sensitive information like credit card info, it automatically mark that form and auto-suspend the account. 

Anyway, I have removed the auto-suspension on the account and whitelisted the forms. You should be able to view your forms.
