How to make a voting form ?

I am helping a local school set-up an online voting system.

In this case for sport team captains, but it could be for any occasion such as favourite team, food or class captain.

To support a strong, fair voting process, they would like both an image and name to be displayed.

For the voting component, they would like to use a ranking or 'preferential' voting process. 1 for first preferred candidate, 2 for second choice and so on.

Voters can only choose a ranking once. That is, in a candidature of 5 people, voters can only choose the number 1, once, numer 2, once and so on.

The sample I am developing is using car names.

the number of candidates in any future election could be from 2-10 or even more.

If I understand correctly your post, you are trying to build a voting system, I see you were able to add an image and the title as well as the options in your form, I would suggest you to take a look to this widget in order to have a better selection in the ranking you want to show in your form.

I would like to be able to have users nominate their selections in rank order 1-5 (or more)

I would like an image (with a label or caption) to be used to assist the user clarify their choices.

My sample form shows 2 approaches I have tried.

1. using  an image and a drop down box - this is not suitable because the user can repeat a ranking: I need all rankings to be used without any repeats

2. The drag and drop approach at the bottom of the form would be suitable IF I could include an image wth each label.

However, your approach allows the user to make the same choice on each occasion. 

For example I can choose Mercedes 3 times whereas I am wanting to have:

1. 5 choices available in question 1 - the chosen option then leaves the available choices for . . 

2. question 2 has 4 options available  - not including the choice made in question 1

3. question 3 has 3 options available - not including the choices made in questions 1 and 2

and so on, allowing all 5 options to be chosen in order of preference - without any option being chosen twice

If you want to hide selected option for incoming question, you can add images to the options on "Radio Button" field. By doing this, if you want to ask more than one question, you can repeat these steps for every question.

To do that:

1. Add "Check Box" or "Radio Button" to your form.

2. While that field selected, click to "Options".

3. In Options, you will see "option 1", "option 2", "option 3" by default, every line means one option. Here, you can paste HTML image code.

Above, yellow marked area is the URL of the image. Just paste the image URL there, it will be appear on that option.

On blue marked areas, you can adjust the size of the image by changing px values.

On purple marked area, you need to identify every image for conditions can see their value. Here, I named that image as a "Answer 1".

I managed to input the html as you suggested but I cannot see how the chosen option does not appear in the next step.

All I am trying to achieve it to allow the user to rank images in order of preference without giving the same ranking more than once. That is, 5 images to be ranked 1-5 in order of preference chosen by the user.

I do not mind if this is by:

dragging images into a preferred order

OR

using a drop down list

OR

starting with 5 images in 'step 1' - whereby the first chosen image is not shown in step 2, the image chosen in steps 1 and 2 are not shown in step 3 and so on.

Unfortunately, there is no workaround for hiding the selected option on the next question. I have suggested this method, because the user can review all of the questions and this will avoid users to choose same choice for other questions.
