I just want the form to be center aligned rather than left page aligned.

Since there is another section on the left side where the form was, you cannot canter positon the form at center of the page.
You may want to check on your web page builder/creator and allot a section to center the embedded form.
If you check the form using its URL http://www.jotform.us/form/53476490323154 it was already centered on the browser
