the form on this page seems to work fine and we get the notifier email but the auto-responder is not going out - http://www.healthworksclearlake.com/np.html
please help get this working again

You are currently using a custom email address as the sender email of the form. Please try switching it back to our default and check if it's still not working for you. But if it's working, then your email doesn't support the method. I would suggest adding it as SMTP instead of the verification:
