    why does it say my forms are disabled?

    I have checked your account and it appears to have been auto-suspended by our anti-phishing system.

    This usually happen when the system detects a form with suspicious fields an labels that pertain to collection of sensitive user information such as login information and credit card information.

    The forms that triggered this in your account are

    I have checked the form and it appears the word Pass triggered the false flag, this was interpreted as a password field. Kindly revise the label of the form  and text-field to allow for reactivation of your account
