move from into paid account

I need to move a form from my personal account (pattykrakow) to my business paid account FIT_Forms.

The form is called 
DRAFT Self Qualification for FIT Credited Internship

I have checked an it appears the mentioned form is already under the paid account, could you kindly log into the account and confirm if you can indeed see the form with that title.
