You will need to check out your email templates to see how they are set. Seems that you have (at some point) added:
A1 Class Pre Requisites - Foundations - Teeters & Weaves - Contacts & Sequencing
PayPal - Choose Class
after the rest of the fields, but we would be happy to look into this for you and help you resolve the same once you tell us what is the exact form that you are having this issue with.

Thank you for the link. I took a look at this form: http://www.jotformpro.com/form/53475766617973 and it seems to be OK, but the other one: http://www.jotformpro.com/form/53056796889982 seems to have wrong field tags in the email template. I would suggest removing those.
Actually the best way to do this quickly would be to remove the current autoresponder and just add a new one in its place.
This way it would automatically get the right names of each field so that you do not have to do it manually. After that, you can simply make the changes at the top - the red letters.
