You can add images to your form using the Image field under the Form Tools : 

Once added you will get this window, select the way you will add the image :

Are you referring to this form - http://www.jotformz.com/form/53484107551656? Do you mean that the upload field in your form is not working in mobile? I tried to access the form in an android device but I was not able to replicate the issue. I was able to select a file to upload using my device. 

Are you still encountering this issue? If so, may I know what device are you using when you encounter the issue? If you have embedded your form in your website, please provide to us the exact URL where you have embedded your form so we can further check the issue.

Esse formulário desenvolvi pra um aplicativo para celular não para site, então a utilizar o APP no smartphone ele funciona perfeitamente mas a opção de upload de fotos pelo celular não funciona aperta e não abri para selecionar foto.

As David has suggested the Filepicker Widget is the best way to attach and upload photos from a mobile device.

It should allow you to tap the upload field at which point you can find a photo in your photo library or take a new one to which would then attach the image depending on your preference.

If you are still having issues with using the upload field on your mobile device then could you please tell us which smartphone model you are having this issue on?

If you have having trouble on Android, try using FireFox as your browser.  Both the stock web browser and Chrome have trouble uploading file through the forms.  However, FireFox seems to work on almost every device.

Thank you, I have downloaded the app and I'm not able to upload a file, when I click on the upload button the app is closed suddenly, is this the same issue you are getting ?

And then, I'm able to upload the file, see my screenshot below:

Please, try again re-embedding your form and let us know if it works, if not, please could you check your application, it could be an issue related to it.

As per testing the direct link of the form in a mobile device, the upload field is working fine. JotForm forms are meant to be used on browsers (mobile or desktop). Using a browser in an android device, the upload field works as intended. However, on your mobile app, it does not work. There might be issues on how you have embedded the form in your app. It is highly likely the software you used to create the app does not permit html forms from accessing the device's file system. I would suggest  you to contact the developer of the software you have used to create the mobile app for further assistance.
